1. There is a $150.00 non-refundable deposit required to hold the date and time. 

2. An additional travel fee is required - In San Francisco:  75.00. Outside of San Francisco: 150.00.  ANYPLACE outside of the Bay Area and/or the state will be discussed during the consultation call.

3. SUBMITTING THIS FORM IS NOT AN INDICATION THAT THE DATE IS CONFIRMED.
